How to Enroll
To enroll in your benefits, please follow these steps:
- Determine the Benefits You Want — Review this booklet to determine which plan(s) you want to enroll in or decline for this year.
- Make Your Elections — How you enroll depends on the benefit (see below).
- Review Your Confirmation Statement — Once you complete your enrollment, you’ll receive a confirmation statement. Print and review it carefully to ensure it reflects your desired elections.
- Provide Required Documentation — If you enroll a spouse and/or children in your coverage, you will need to provide required documentation (see below).
For medical, prescription drug, supplemental medical, dental, vision and tax-savings accounts

Required Documentation
If you enroll a spouse and/or children in your coverage, you will need to provide the following documentation:
Provide proof of dependent status — If you enroll a spouse, same-sex domestic partner and/or children in medical coverage, you must provide proof of their eligibility (e.g., marriage license, birth or adoption certificate, etc.) by the deadline. Otherwise, they won’t have coverage. After you enroll, look for a letter from the Dependent Verification Center in the mail with further instructions.
Complete the spousal surcharge affidavit — If you enroll a spouse or same-sex domestic partner in medical coverage and you select “spousal surcharge does not apply,” you must complete this form by the deadline. After you enroll, look for a letter from the Caterpillar Benefits Center in the mail with further instructions.
