NETWORK PROVIDER CHANGE REQUESTS

Many times Caterpillar is contacted by contracted provider offices who need to make changes to their practice.
Examples:

  • Office location address or adding additional locations for a contracted provider
  • Office payment address for a contracted provider
  • Billing Tax Identification change for a contracted provider

REQUIREMENTS FOR THESE CHANGES

Change Documents Needed
  • Office location address change
  • Add additional office locations for a contracted provider
  • Office payment address change
  • Tax Identification change for a contracted provider

In order for changes to be made timely, the request must be sent no later than 15 working days prior to the effective date of the change.

Be aware that Tax Identification changes will affect the provider contract and may require additional documents to be completed.
