NATIONAL PRACTITIONER DATA BANK (NPDB) REQUESTS

The Caterpillar Healthcare PPO NetWork requires physicians and mid-level providers  to complete a self query of the Data Bank when completing an Initial/New and Recredentialing application. The self query of the National Practitioner Data Bank report is used (along with data from other sources) when considering a practitioner’s credentialing application.

Caterpillar reserves the right to request providers, other than physicians, to complete the self query of the National Practitioner Data Bank as needed.

WHAT'S THE NATIONAL PRACTITIONER DATA BANK?

The NPDB (National Practitioner Data Bank) and HIPDB (Healthcare Integrity and Protection Data Bank) are information clearinghouses created by Congress to improve healthcare quality and reduce healthcare fraud and abuse in the U.S. Collectively, the NPDB and HIPDB are referred to as the "Data Bank."

To access the self query of the National Practitioner Data Bank, click the link below and choose “search on myself”:

Once the self query of the National Practitioner is completed online, a notarized copy must be mailed to the National Practitioner Data Bank in order to receive the report. Here are the addresses that will accept the notarized copy:
